The C19 Group has received numerous emails over the past three months from employees reporting COVID-like symptoms. In order to streamline the process, we request employees to fill out the form below if/when an employee begins to experience the COVID-like symptoms and/or is directly exposed to someone suspected or infected with C19. By providing these answers initially, we will be able to access and react to an individual's situation in a more efficient and effective manner.
